Choosing an SDS-Max drill bit
SDS-Max bits are for big holes and deep holes in concrete and masonry. The range here is ITM: X-Cutter SDS-Max drills from 3/8" up, and 3-1/8" concrete tunnel drills in 22" and 39" lengths. Some X-Cutters are very long, up to 47" x 52" in the titles, for going through thick walls and slabs.
Drilling tips
Run an SDS-Max rotary hammer with hammer action on and let the tool do the work. Leaning harder does not cut faster and it heats the tip. Back the bit out from time to time to clear dust, especially in deep holes. If it stops advancing, you may have hit rebar. Stop and check before forcing it.
X-Cutter titles list two lengths, so check both against your depth and the reach of your tool. For holes bigger than 3-1/8", look at SDS-Max core bits.

Common questions
Can I use SDS-Max bits in a hammer drill with a regular chuck?
No. They need an SDS-Max rotary hammer. The shank is made for that chuck and the hammer action is what lets the bit cut concrete.
How long do these bits get?

What if I need a hole larger than 3-1/8"?
Use an SDS-Max core bit. Core bits cut a ring instead of the whole hole, which is easier for larger diameters, and the range goes up to 6".
